What to expect from the Developmental Care Coordination program?
Program Steps
Outreach – Parents give their consent
CCRC Case Specialist connects with family to provide program information and program steps. Family makes a decision to continue and sign the Release of Information (ROI) form.
Care Coordinator connects with family
A CCRC Care Coordinator partners with families to learn more about the family’s needs, give an in-depth overview of the great benefits of this program, and go over the screening tools that will be conducted over the phone. If preferred, an email with the developmental screening link is sent.
Developmental Screenings
Developmental screenings are completed by a smartphone or desktop computer. Families will complete five developmental questionnaires such as the Ages and Stages Questionnaire -3, Family Needs Assessment, Adverse Childhood Experiences, Pediatric ACEs and Related Life Events Screener, and Positive Childhood Experiences questionnaire.
Screening Results
Once developmental screenings are completed a Care Coordinator will reach out and go over screenings results. Based on those results and discussion, the Care Coordinator provides resources to address family needs.
Connect Families to services
If families receive referrals and help navigating community services and developmental interventions. Families will receive new developmental screenings based on the child’s age to help track and support upcoming milestones.

By providing developmental screenings and resources needed for family well-being. Accessing resources can be stressful or hard to find, this is why dedicated Care Coordinators are here to help.
What can the Care Coordinator help with?
Care Coordinators follow-up with families in intervals of 15, 30, and 45 days to ensure families can access community resources, assistance in navigating and locating community resources, assistance with developmental screenings, and addressing barriers.
Who can participate?
Families receiving subsidized child care through CCRC can opt into participating in the Developmental Care Coordination program.
